About the Role
As a Bilingual Community Health Worker, you will serve as a bridge between healthcare providers and the community, helping individuals overcome barriers to accessing healthcare and social services. This role focuses on serving Spanish-speaking communities in the Hampton Roads region.
Responsibilities
- Conduct health screenings and assessments in community settings
- Provide health education on chronic disease prevention and management
- Assist clients with healthcare navigation and appointment scheduling
- Translate materials and interpret during medical appointments
- Build relationships with community organizations and faith-based groups
- Track and report program outcomes
