Position Summary
We are looking for a dedicated Behavioral Health Case Manager to coordinate care for clients with co-occurring mental health and substance use disorders. This part-time position (20-25 hours/week) offers flexibility and the opportunity to make a meaningful impact.
Duties
- Develop and monitor individualized treatment plans
- Coordinate services across multiple providers
- Conduct regular client check-ins and progress assessments
- Advocate for client needs with insurance companies and service providers
- Maintain compliance with state and federal documentation requirements
Requirements
Masters degree in social work, counseling, or related field. Licensed or license-eligible (LPC, LCSW). Experience with co-occurring disorders preferred.
